ok, this is seen all the time, and I've come close to writing it too I think:

a lot of odd things seen in writing come from people trying to write down the sounds spoken

A classic failure of cliché comprehension is this gem uttered by GW Bush:

conformation (in place of confirmation, the rite, pretty good I think)
plug up the courage

to persevere and conker
with all excessories (item for sale)
defiantly (used in place of definitely)
